Macular Hole: Why Straight Lines Look Wavy and How It Is Treated

Quick answer

A macular hole is an opening in the tiny retinal area responsible for sharp central vision and reading. It may cause central blur, a small missing spot or wavy lines. A retinal examination and OCT show its size and stage, guiding a choice between monitoring and surgery.

Symptoms you may notice

Words may seem missing in the middle of a line, door edges may look wavy or fine detail may be harder to see. The hole usually does not cause pain. Covering each eye in turn may reveal a difference, but it cannot replace an examination.

How is it different from an epiretinal membrane?

An epiretinal membrane is a thin layer that can tug on the retinal surface and distort lines. A macular hole is an opening through the central retinal tissue. Symptoms can overlap, so OCT helps the ophthalmologist distinguish the two.

How is the diagnosis confirmed?

A retinal specialist examines the back of the eye and uses OCT, a quick, painless scan that shows retinal layers in cross-section. The specialist measures the opening, checks how far it extends and examines the other eye before explaining what it means for you.

Does every hole need surgery?

Some small or early holes may be monitored with repeat scans if the ophthalmologist considers that safe. For larger or more advanced holes, vitrectomy with peeling of a thin membrane around the hole is a common option. The decision depends on the hole, vision and duration, not one symptom alone.

Recovery after surgery

A surgeon may place a gas bubble inside the eye to support healing and may ask you to hold a certain head position for a period. Ask about flying, anesthesia and restrictions before surgery, and keep follow-up visits. Do not change your drops or positioning instructions yourself.

Frequently asked questions

Does a macular hole cause pain?

Usually not. The main sign is a change in central vision, although severe pain or redness should also be assessed for another cause.

Will vision return to normal after surgery?

Vision may improve and the hole may close, but recovery varies and some distortion or blur can remain.

Can I delay an examination if the blur is mild?

Do not delay assessment of new central distortion. The ophthalmologist determines urgency after checking vision and the retina.

Do I need an OCT scan?

OCT often helps confirm and measure a macular hole, but the ophthalmologist decides which tests are needed after examination.

This article is for education and does not replace an eye examination. Sudden vision change, severe pain, an eye injury or another emergency sign needs urgent assessment.
