Epiretinal Membrane: Why Lines Look Wavy and When Surgery Is Considered

What is an epiretinal membrane?

The macula is the small central part of the retina used for reading and fine detail. A thin layer can sometimes form over it and gently pull on its surface. It is also called a macular pucker or premacular membrane.

Symptoms you may notice

The condition may cause no symptoms. If it affects vision, straight lines may look wavy, objects may seem different in size, or reading may become blurry or difficult. It is usually painless.

Examination and imaging

The ophthalmologist examines the retina, often after dilating the pupil, and uses OCT to image the macular layers. The scan is interpreted alongside vision testing and symptoms to distinguish a membrane from other causes of distortion, including macular degeneration.

Does it need surgery?

If vision is good and symptoms are limited, periodic examinations may be enough. No eye drop removes the membrane. If blur or distortion substantially affects reading or work, a retinal surgeon may discuss vitrectomy and membrane peeling. Visual improvement varies, and surgery has risks.

When should I seek prompt care?

Gradual distortion should be assessed, but sudden worsening blur, many new flashes or a curtain at the edge of vision is not typical of a membrane and may signal another retinal problem. Seek urgent care.

Frequently asked questions

Can an epiretinal membrane cause blindness?

It can reduce central vision to varying degrees, but many mild cases remain stable and do not need surgery. Monitoring checks for change.

Can drops treat it?

No drops remove the membrane. Management is observation or surgery for selected cases that affect daily life.

Is OCT painful?

No. The scan is quick and does not touch the eye. Drops may be used to dilate the pupil for part of the examination.

Does everyone need surgery?

No. Symptoms, vision and OCT findings are considered together before choosing monitoring or surgery.

Medical note: This guide is for education and does not replace an eye examination. Sudden vision loss, severe pain or an eye injury needs urgent assessment.
