Keratoconus: When Are Glasses Enough, and When Is Cross-Linking or Another Procedure Needed?

Keratoconus is more than "high astigmatism"

Keratoconus is a corneal ectatic disorder in which the cornea becomes biomechanically weaker, thinner and more irregular. Patients may notice increasing astigmatism, frequent prescription changes, ghost images, glare or distorted vision even when standard visual acuity appears acceptable.

The disease can remain relatively stable in one patient and progress in another. A diagnosis alone does not determine treatment. Management depends on whether progression is occurring, corneal thickness, age, symptoms and topographic or tomographic findings.

Glasses and contact lenses improve vision; they do not stop progression

Early keratoconus may be managed visually with glasses or soft contact lenses. As irregularity increases, rigid gas-permeable, hybrid or scleral lenses can provide better optical quality.

These devices improve the image reaching the eye but do not mechanically strengthen the cornea. That is why serial imaging is important, especially in younger patients or anyone with rapidly changing refraction.

When is corneal cross-linking considered?

Corneal cross-linking uses riboflavin and ultraviolet-A energy to increase biomechanical bonding within the corneal stroma. Its central role is to reduce further progression in suitable eyes with documented or strongly suspected progression.

A 2025 systematic review and meta-analysis reported better visual outcomes in some patients receiving customized PRK plus CXL than CXL alone. This is a personalized strategy, not a routine recommendation for every keratoconus patient; corneal thickness and tissue-removal limits are critical.

When is corneal transplantation needed?
