Migraine Visual Aura or an Eye Emergency? How to Tell the Difference
Quick answer
Migraine aura may look like zigzag lines, flashes or a spot that gradually expands over minutes and then fades. It can seem to affect one eye even when it involves the visual field of both. Sudden loss of sight in one eye or a first, different episode needs urgent assessment, not self-diagnosis as migraine.
What can a typical aura look like?
A small bright spot or zigzag may slowly move or expand, with or without a headache afterward. Nausea or light sensitivity can accompany it. A familiar pattern does not prove the diagnosis, especially during a first episode or when symptoms have changed.
Is it in one eye or in the visual field?
A person may think the flashing is in one eye because one part of the scene is affected. Covering each eye in turn during an episode may offer a clue, but it cannot rule out an eye problem and must not delay emergency care for vision loss. It is not a diagnostic test.
When should you not assume it is aura?
A sudden curtain or blackout in one eye, many new flashes with floaters, or a deficit that does not resolve needs urgent assessment. Seek emergency help for weakness or numbness on one side, speech difficulty, confusion or an unusually severe headache.
What does retinal migraine mean?
Retinal migraine is rare and describes temporary visual symptoms confirmed in one eye after more important causes, such as retinal or circulation problems, have been excluded. A headache with blur is not enough to label it retinal migraine. The appropriate eye or neurological assessment depends on the signs.
What should you do during symptoms?
Stop driving or using machinery, sit somewhere safe and note when symptoms began, how long they last and whether a headache followed. Seek urgent care for a first or different episode or one that does not resolve as usual. Discuss recurring, previously assessed episodes with your treating clinician.
Frequently asked questions
Does migraine aura affect only one eye?
It can seem that way, but it often affects the visual field in both eyes. Sudden change in one eye needs assessment before assuming migraine.
Can aura happen without a headache?
Yes, visual symptoms can occur without headache, but a first or different episode still needs appropriate assessment.
Can I drive during flashing vision?
No. Stop somewhere safe and wait for symptoms to resolve. Do not drive while vision is affected.
Should I use eye drops?
Eye drops do not treat migraine aura. Do not start a treatment until the cause has been assessed.
This article is for education and does not replace an eye examination. Sudden vision change, severe pain, an eye injury or another emergency sign needs urgent assessment.
