أما الحول الظاهري فالعينان تكونان مستقيمتين، لكن شكل الأنف أو ثنية الجلد بجواره قد يعطي انطباعًا بأن العين داخلة للداخل، خصوصًا في صور الموبايل. الفحص البسيط للضوء على العينين واختبار التغطية عند الطبيب هما اللذان يفرقان بين الحالتين. لا تفترض أن الأمر شكلي لمجرد أن الطفل يرى ويتحرك طبيعيًا.

كيف يتم الفحص؟

الفحص لا يقتصر على النظر إلى صورة العين. الطبيب يراجع تاريخ بداية الانحراف، ويفحص كل عين على حدة بما يناسب عمر الطفل، ويقيس حركة العينين، ويستخدم اختبار التغطية وانعكاس الضوء. غالبًا تُقاس درجة النظارة بعد قطرات توسع الحدقة لأن طول النظر أو الاستجماتيزم قد يكونان جزءًا من السبب.

Childhood Strabismus: When It Needs Assessment and How It Differs from Pseudostrabismus

What is the difference between true strabismus and a photographic appearance?

In true strabismus, the eyes are not directed toward the same target. One eye may turn toward the nose, toward the ear, upward or downward. The turn can be constant, appear with tiredness, show up in photographs or occur only at certain distances.

With pseudostrabismus, the eyes are actually straight, but the bridge of the nose or a skin fold near it creates a crossed-eye appearance, especially in phone photos. A light-reflex check and cover test during an eye examination distinguish the two. Normal activity or apparently good vision does not prove that a turn is only cosmetic.

Signs families may notice

Notice whether the eye turns most of the time or only at times, and whether it is more obvious during concentration, fatigue or bright light. A child may close one eye in sunlight, turn the head, hold objects very close or appear not to use both eyes together. These observations help the clinician but do not identify the type of strabismus by themselves.

A constant eye turn at any age deserves assessment. A turn that begins suddenly in an older child or adult, or occurs with double vision, severe headache, vomiting, a drooping eyelid, generalized weakness or a clear change in vision needs same-day urgent evaluation.

How is a child assessed?

Assessment is more than looking at a photograph. The clinician asks when the turn began, checks each eye in an age-appropriate way, measures eye movements and uses cover and light-reflex tests. Refraction is often measured after dilating drops because farsightedness or astigmatism can contribute to the alignment problem.

The examination may include an amblyopia assessment, front-of-eye and retinal examination, and extra testing when the movement pattern or symptoms are unusual. The goal is not only to name the turn, but to determine whether it affects binocular visual development or reflects another vision problem that needs treatment.

Treatment is not one operation for every child

The plan depends on the type and cause, the child's age, the size of the turn and how each eye functions. If the correct glasses improve alignment, they can be an essential part of treatment. If amblyopia is present, the clinician may prescribe patching of the stronger eye or, in selected cases, drops to encourage use of the weaker eye. Amblyopia treatment protects vision, but it does not correct every form of strabismus or replace the rest of the plan.

Eye-muscle surgery is discussed when the turn is persistent, affects binocular use, or is not adequately controlled with glasses and other measures. There is no one correct age for every child and no guarantee that one operation solves every case. Timing and technique come from repeated measurements and the child's examination, and some children need follow-up or later adjustment.

Strabismus and amblyopia are related but not the same

Strabismus is an alignment problem, while amblyopia is reduced visual development in one or both eyes during childhood. Strabismus can lead to amblyopia in some children, but not every child with strabismus has amblyopia and not every case of amblyopia is caused by strabismus. Vision in each eye should therefore be checked even if glasses or surgery make the eyes look straighter.

What can parents do now?

Take a short photo or video when the turn appears and note whether it happens at a particular time, during illness or with fatigue, then bring it to the visit. Do not begin exercises or patching on your own, and do not delay assessment because the turn is intermittent. Glasses, patching and surgery cannot be selected from one photograph.

If the clinician confirms pseudostrabismus, that is reassuring, but follow the recommended review plan. If a new sign or a clear change appears, arrange reassessment rather than relying on photographs or someone else's experience.
